Shoud i create multiple document to store form data or create nested json in single document ?

My use case is to store form data. I attached a picture of form which describe form layout.

I need to store all fields in Product collection. Form has a Location field which can be multiple, i am confused how this field will be stored.

1 ) Should i store complete data in one collection as nested json 

2 ) Create another Location and LocationConnections collection and store all location in Location collection and provide each location connection with product in LocationConnections.

I need to know the best practice that should be followed.

As i worked on SQL, i store it in different table and provide foreign key
