Objectives: To examine the frequency, intensity, and predictors for symptoms in the last seven days of life among patients who were able to communicate and died in an acute palliative care unit (APCU).

Results: A total of 203 of 357 patients died. The proportion of patients able to communicate decreased from 80% to 39% over the last seven days of life. ESAS anorexia (P = 0.001 in longitudinal analyses), drowsiness (P < 0.0001), fatigue (P < 0.0001), poor well-being (P = 0.01), and dyspnea (P < 0.0001) increased in intensity closer to death. In contrast, ESAS depression (P = 0.008) decreased over time. Dysphagia to solids (P = 0.01) and liquids (P = 0.005) as well as urinary incontinence (P = 0.0002) also were present in an increasing proportion of patients in the last few days of life. In multivariate analyses, female sex, non-Hispanic race, and lung cancer were significantly associated with higher ESAS symptom expression (odds ratio > 1).

WDFW shellfish managers are considering an alternative digging schedule for Copalis and Mocrocks beaches beginning in 2024. The proposed change would open each beach for multiple days rather than alternating openings between areas every other day. For example, during a four-day tide series, Mocrocks would be open Friday and Saturday and Copalis would be open Sunday and Monday. Proposed dates would provide consecutive open dates on each beach in a way that spreads digging opportunity evenly.

The approach to offering consecutive open days may reduce confusion as to which beach is open, create more digging opportunity when only one beach is available for harvest, allow for increased digging success due to beach familiarity, and reduce travel to and from open beaches. In this study we aimed to develop a protocol that enables the creation of multiple deletions in a few days using a combination of dsDNA lambda Red recombineering and flippase recombinase mediated excision of integrated antibiotic markers. The developed methods were used to create multiple combinatorial deletion mutants based on data from recently published transposon library selections38. Combinatorial deletion mutants were found to be resistant to high concentrations of acetate, NaCl and a combination of the two, which is a condition that is often prevalent in high cell density fermentations.

In this study a protocol was developed that enables the construction of seven deletions in as little as seven days (Fig. 5) based on the combined action of lambda Red recombineering and flippase recombinase mediated excision of integrated antibiotic cassettes. The developed protocol can be used in combination with the Keio collection of single gene deletion strains and can be easily combined with a similar system based on Cre-recombinase mediated excision of antibiotic cassettes. With proper consideration of the genomic locations of desirable modifications, it may furthermore be possible to combine the dsDNA recombineering used in this study with ssDNA recombineering, which could facilitate even faster engineering of multiple genomic alterations.

Estimated time for making up to seven markerless deletions using either the integrated 2:1:2 or 1:1 system in strain SIJ488, the one-plasmid procedure using pSIJ8 or the standard two plasmid based procedure.

Background. Azacitidine (AZA) and venetoclax (VEN) combination is approved in previously untreated acute myeloid leukemia (AML) patients ineligible to intensive chemotherapy (IC). However, AZA-VEN is associated with toxicities that often requires dose reduction (or discontinuation) and may limit triplet combination tolerance. In our study, we evaluated efficacy and toxicity of VEN exposure reduction by limiting VEN intake to the concurrent days of AZA administration (7+7 scheme).

Methods. We retrospectively analyzed data from untreated AML patients ineligible for IC who received AZA for 7 days associated with VEN for only 7 days from the 1st cycle every 28 days. Response to therapy was evaluated according to ELN-2022 criteria's and comparisons between cohorts were determined using Chi2 test. Time-to-event analysis were assessed using the log-rank method, with logistical regression analysis and Cox proportional hazard modeling to determine the effect of variables.

Among 56 CR/CRi patients, 61% had dose reduction mainly due to grade III/IV cytopenias: reduced treatment duration to 5 days in 97%, VEN dose reduction to 200mg/d in 50% and a 5-weeks interval between cycles in 29% of patients. Estimated median OS in patients with reduced dose was 25.8 months.

Discussion. Limiting VEN exposure to the concurrent days of AZA administration (7+7 scheme) induced similar response rates than the recommended VEN administration for 28 days. The 7+7 scheme, applied to the same population than VIALE-A trial, compared favorably to the 7+28 usual dose. Even with reduced VEN exposure, significant toxicity occurred during cycle 1 and subsequent cycles. In responders, subsequent dose reduction of VEN seems associated with favorable outcome. Our results provide rationale for reduced AZA-VEN scheme used in current clinical trial evaluating triplet combination.

Most of our time reckoning is due to the movements of the planets, Moon and stars. Our day is equal to one full rotation of the Earth around its axis. Our year is a revolution of the Earth around the Sun, which takes 365 and days, which is why we add an extra day in February every four years, for a leap year.

Finally, once the Romans began to conquer the territory influenced by Alexander the Great, they too eventually shifted to the seven-day week. It was Emperor Constantine who decreed that the seven-day week was the official Roman week and made Sunday a public holiday in A.D. 321.

The weekend was not adopted until modern times in the 20th century. Although there have been some recent attempts to change the seven-day week, it has been around for so long that it seems like it is here to stay.
