Translate word by word?

Do we have to translate the meaning of every word? or, just express the meaning of a sentence?

I found sometimes, I could not re-assemble these words as a fluent Chinese sentence if I translate all of them, and I prefer to reorganize in my words. 

Any suggestion? Maybe both my English and Chinese are poor...:(

I guess it should be the latter one, just express what you catch in English and rewrote completely in Chinese.. 

At least I was doing so. Don't know if there is any better means when it comes to translation.. XD

If I did translate every word, it would have been too slow...

any suggestions from others?
