Description: Supervise and ensure Health Care Coordinators in fulfilling their roles and responsibilities. Support ICF Assistant Managers. Meet with Health Care Coordinators and ICF Assistant Managers weekly to review medical issues. Act as liaison between physicians, pharmacists, medical supply companies, insurance companies, etc. as needed. Review on a weekly basis and document on those targeted individuals who require close supervision by medical personnel. Review the consumers’ medical records and complete medical concern sheets quarterly and ICF books monthly to ensure accurate documentation and optimum quality of care. Intervene in any necessary medical concerns. Identify training issues and support staff with training as the need arises. Attend 90 day Psychiatric Medication Reviews as needed. Attend Nurses Network Meetings facilitated once a month by PCHC and Managers’ Meetings facilitated by JEVS. Attend ISP Meetings, Team Meetings and Quarterly Reviews as necessary. Create Health Care Plans for all medically involved individuals. Review all medical history summaries for accuracy and content. Additional responsibilities as needed.
REQUIREMENTS
Registered Nurse or Licensed Practical Nurse. Valid Driver’s License. At least two years experience working with individuals who have developmental disabilities. Supervisory experience is preferred. Familiarity with ICF/MR, CRR and/or CLA residential programs is desirable.
