What is the exact format for .gcp files for RPC cameras?

I am processing BJ-3 stereo images (RPC cameras) and have 5 GCPs with
lon/lat/ellipsoidal height from a reference DEM.

I need to create a .gcp file for bundle_adjust. The documentation
shows GCP files being passed to bundle_adjust but does not specify
the file format.

My current file is:
1 80.455014 29.664273 1165.0
2 80.416808 29.691412 1098.0
3 80.465747 29.715084 1633.0
4 80.436577 29.692909 1224.0
5 80.436012 29.736264 1471.0

bundle_adjust fails with "Cannot find the file: 1", indicating it is
trying to interpret column 1 as a filename.

What is the exact required format? Do I need to include pixel coordinates?
If so, what tool should I use to generate them for RPC cameras?

The format for GCP is here: https://stereopipeline.readthedocs.io/en/latest/tools/bundle_adjust.html#bagcp

There are links to that from various places in the doc including from the first place in the tool page where GCP are mentioned, but likely there could be more such links.
