Lab CLIA number question for electronic claims

Hi,

When posting electronic claims, the HFCA field for box 23 Prior
Authorization Number (CLIA number for lab dx) is not printing on the
form (blank value on the electronic clm file) and claims are denied. I
have tried various form field definitions, copied below (including the
ECLAIM one from Medpipes website), but to no avail.

ECLAIM:

[Bill:Diag2,0.48,,1,Left,Font 1]
[Bill:Diag4,3.23,,1,Left,Font 1]
[facility:CLIA,5.250,0.000,3.000,LEFT,Font 1]
;

PROCLAIM:

[Bill:Diag2,0.48,,1,Left,Font 1]
[Bill:Diag4,3.23,,1,Left,Font 1]
[Bill:Ins1_Prior,5.25,,3,Left,Font 1]

Neither the "facility:CLIA" nor the "Bill:Ins1_Prior" yield any value
for that box 23 field. I cannot determine whether the issue is with
the electronic claim form definition itself, or whether some field is
not entered in the bill itself. Any suggestions are much appreciated.
