OP-80a Paper Tape Reader Question

I just bought an OP-80a bare board. It only came with the optical sensor. There are no markings on the board for component values, IC names, etc. There's not even an indication of polarity for the sensor. Does anyone have a BOM for this board, maybe a component layout diagram and a template for bending the tape guides? I have the manual but it assumes the board is already built.

Hi,

I’ve been trying to get the basic kit from the guy in the UK for over a year!  He doesn’t sell to the USA anymore. 

Can you give me a high-resolution scan of the top and bottom of the board? 

Or maybe you can create a KiCAD schematic and board and Gerber files and I can post them on my website so people can build their own OP-80A paper tape readers.
