Do most of the big casino-hotels have an "a la carte" breakfast
restaurant?
My Frommer book and the net seem to focus on the buffets (which is
normally my kind of meal). Any help would be greatly appreciated.

Every casino has a coffee shop, and for breakfast food they are mostly
interchangeable.
Keep an eye out for specials... there are some amazing deals out
there.

Yes, most of the major casino resorts have at least one place
where you can get a regular non-buffet breakfast. There's also
a restaurant next to the New New New York that serves a great
breakfast at a modest price, but the name of the place escapes me.
You will have no problem finding a place to have breakfast.

The other replies are right. Virtually every casino has a basic coffee shop
or food court. I've eaten in the IP coffee shop and it had ok basic food.
Don't go to Denny's. They have crowds of people looking for a basic
breakfast and and often a long wait. On top of that they don't offer promo
prices so the Grand Slam runs something like $7.50.
I hate Denny's anyway.

All casinos will have a coffee shop with a so-so to good breakfast. The
Teahouse Coffee Shop at the IP is good for breakfast since you're
staying there, but don't go out of your way for it.
I was impressed with the breakfast at Ellis Island. Cheap and no line
at all. Only problem seems to be that their nonsmoking section is closed
sometimes.
For more of a breakfast type place, I've been to Blueberry Hill on
Flamingo, which I think is a bit above IHOP type places and the casino
places.

Go to the Victorian Room at the Barbary Coast and order the skillet
breakfast. It's an iron skillet with fried potatoes, onion, green pepper,
and your choice of meat. Then it's topped with 3 eggs cooked like you want.
Plus a side of toast. Cost is $5.99 (I think).

If you don't mind leaving the strip you might want to try Blueberry
Hill Family Restaurant at 1545 E Flamingo Rd. It's about 2 miles east
of the strip. There are several in the area but we always go to the
one on Flamingo and have for years. It's pretty much a locals place
so the prices are down. The food and service are good and it's a very
friendly place.

The Barbary Coast, which is very close to the IP, has a good breakfast
menu at their Victoria Room rest. - here's a sample!

Also, I believe they have a very good early-bird breakfast special -
can't remember exactly. Hope this helps.
Ellis Island has a $5 steak and eggs special - same off-the-menu
dinner steak, but with eggs instead of potato and veggies. Ellis
Island is just off the Strip behind Bally's on Koval.

Most of casino/hotels have decent breakfast at their cafes. However in
some you'll pay about the same or even more than the buffet. Haven't
been to IP cafe for a long long time and it was fair at best. But if
you stick to eggs, bacon toast you might be ok. Caribe cafe at Mirage
has always been good for breakfat as is the cafe in Caesars. IP has a
buffet but it's not very good. Flamingo cafe is ok.

I eat at the Flamingo Blueberry Hill on many a trip. It's a great
place to take folks who need to get away from the clatter of casinos
for awhile. Good food - at least the basic breakfast items.
But it's not really a mom and pop place. More like a local (and
better!) version of an IHOP.
If you're lookin' for something cheaper and closer to the IP, as others have
mentioned, I'd also go with the early breakfast specials at the Barbary Coast.
