I very much doubt that Conway was the first and only person in
history to think of this bit of wordplay (though probably he was
the first to get it into a book title).

The ngrams will also tell you that the 1960s saw a very steep rise
in the appearance of "cunnilingus" itself in print. Previously it
had been largely confined to medical/psychological and pornographic
literature. In the 60s the word became far more widely known and
acceptable in public discourse. The result was that people who thought
of it (and I have no doubt there were many) had opportunities to
use it in print.

I first heard the term when a friend from San Francisco used it some 45 years ago. Later he was sent to prison for a different reason.
