The only reference I could find to anything close to a Chad Keller was from a
CNN list of passengers from American Airlines Flight 77 which originated in
Washington and crashed into the Pentagon. Here it is as listed at CNN.com:

Yes, one and the same. Apparently, he also went by Chad, but Chandler
was his given name.

No idea. He was not in the business so it
must have been personal. He was from
California so he might have been close to
anyone on the show. And don't forget the
surfing background.
