Found it, courtesy of Youtube! What was really unbelievable was
finding the cover again in Google Images just by searching on the
title, plus the words elementary and textbook.

(includes most of the pages!)

I'm also surprised to find that it was a Golden Book (with a
sophisticated vocabulary, but still, you don't expect such books to be
used as textbooks). The author is Mitchell Wilson and the year was
1959.

This time, you can see some of the pictures (by Cornelius de Witt) in
much more detail without having to click more than once.

Guess what else? The author was married to the famous acting coach,
Stella Adler! She appeared in the 1941 William Powell/Myrna Loy movie
"Shadow of the Thin Man."
