Voyager hit a subspace mine and got flooded with radiation, forcing an evacuation. Just before leaving, Janeway activates the Emergency Command Hologram program, giving the Doctor command of Voyager and a command level officer's uniform. But where are the 4 pips that appeared when the Doctor was daydreaming about being an Emergency Command Hologram in "Tinker,Tenor, Doctor, Spy"?

Near the end, the plant's doctors tell Tuvok that he is suffering from Disphoria Syndrome. In other words, Tuvok thinks he's a woman?
