I remember twice in the first season there was a reference to black/
white. Once was the stones in the bag found with the the two skeletons
in the caves, the other was when Locke and Walt were playing the
boardgame.

I don't remember it coming back up again until we saw Jacob and friend
on the beach, Jacob wearing a white shirt and "Esau" wearing a black
one.

Since I don't have a cross-linked database for Lost like others
here ;) what other black/white references were there in the series?

It was, well still is, a direction to fans to watch in black and white. If
you had done so, you would have seen, via an optical illusion, extra action
in many scenes of every episode, giving you a whole parallel plot which is
other wise hidden. This then explains what you see in the color version.
Because of how the encoding works, you only see this on HD quality versions.
Xvid, DivX, etc, compression destroys the effect.

A lot of what people think are hints of Smokey and other odd CG artifacts
are actually parts of the B&W effects bleeding through. Notice that these
spottings happened in the early seasons, and there a less, if not none, in
the later seasons. This is no more than the technique being refined and
perfected.
