It's been a while since I saw the film, but I think that Heston's
character only begins to suspect Stewart's later in the film. I don't
think that he knew for sure.
But ok, assuming he did, IANAL so please take this FWIW, I think he
could be charged with something like aiding a fugitive or obstruction,
but not for being any kind of accessory to the murder unless you can
make a more direct connection or show he helped plan it or maybe
benefited from it.

It's not clear to me that the others can be charged since I don't
think suspecting some clown is a doctor is a crime.
I think that was part of the irony of the film. Up to that point, the
circus folk only suspect, I think, AFAIR, and I think when Stewart
agrees to help Heston, he is completely giving up the game. It's like
a confession. After that he won't be able to deny that he's a doctor.
But it's been a long time since I saw it.
I wish to repeat, IANAL, so my answers reflect my lack of
understanding. And my memory of the film probably isn't all that good
either.

No. He wasn't there when the clown killed her. He could be taken in
for harboring a fugitive, but they'd only do it if they have some
evidence that the person in question knew.
The clown didn't kill her the doctor did.
