How many of you caught it?

In today's last scene between Chad and Stefano, Chad screams that "It's
over!", turns the opera back on and then points to the ceiling and nods
once.

Metaphorically speaking who / what might the "fat lady" be? When the truth of the paternity comes out?

It has nothing to do with pregnancy...

The "fat lady" is the opera singer. The cartoon sterotype of an opera
singer is a robustly figured woman in a horned helmet singing Das
Rheingold...

I believe I first heard it on an ancient Bugs Bunny cartoon.

The opera is over when the woman in the helmet sings the final aria.

I know the origin of the expression. But there's not going to be a fat woman singing on the show. It represents a climax. I think that will be when the truth comes out that Chad is the father. That might be the catalyst to the serial killer!

David, I think you're over-analyzing this. It was just a cute little moment
where they played a fat woman singing while Chad is yelling it's over. It
was just a little joke between the director and the audience and not really
intended to metaphorically mean anything else.

It's not a metaphor, just a joke. Since Stefano was listening to opera,
they were just playing with it.
