Possible epididymectomy

I have had testicular pain for years, mostly in my right testicle.
The urologist had an ultrasound performed, and said that they found a
"rat's nest" of vein in my left testicle (even though it has less pain
than the right). The left testicle is not really that tender to the
touch, but the right is very sensitive and causes discomfort when
sitting for long or even short periods of time... I could live the
discomfort for the rest of my life if needed. The doctor has
recommended an epididymectomy for both testicles. I have been reading
up on some of David Browns site and saw something that stated sperm
granulomas are developed, or enlarged, in the epididymis after a
vasectomy and that is actually a good thing to help absorb the sperm,
and help decrease the pain of PVP. BTW, does PVP stand for "Post
Vasectomy Pain"? My main question for this site is can an
epididymectomy cause more complications that just a vasectomy? My
thinking is yes, since the epididymis is removed and so are the
granulomas which are needed to help absorb the sperm. Also, is there
a higher chance for a man to develop Dementia over a man who just has
a Vasectomy? I really appreciate any information you can provide.

The granulomas are a response to leakage of sperm. As such they don't really
assit in absorbing sperm. They do "signal" that built up vasectomy pressure
has led to some kind of small blowouts. Usually this was through ruptures,
often in the epidydimous, due to initial sperm build up and pressure. The
granulomas are mmore of a scar reaction tissue. If the Dr. thinks this scar
tissue is causing your pain, then it seems worth a shot. Usually granulomas
settle down sometime after a vasectomy, but if nerves have been involved it
seems possible there is a constant irritation of the nerves going on.
Dementia is not linked to vasectomy!
