I had a vasectomy 9 months ago - and ever since, have had pain in left
testicle. Feels like it's constantly being pinched or pricked. Some
days better, some worse. Been to 3 urologists. First said he could do
nothing. Second said to try 4 Aleve a day for a month, and hot baths
as needed. Didn't really help much. Third did an ultrasound and found
a lump which he thinks is either a cyst or a granuloma, and may, or may
not be, the cause of my pain. He can go in and remove it, during which
time he'd also "redo" the ends of the vas in an attempt to strip away
any irritated nerves. But, of course, the catch is, the surgery could
worsen the pain, and there's no guarantee it will fix it. One doc said
it could take up to 2 yrs for pain to go away, and I'm trying to hold
out, but I'm tired of being uncomfortable, and am considering the
surgery.

Anybody have experience with granuloma/cyst removal? Worth it?

Anybody know of a PVP specialist in California (I'm in the SF Bay
Area)?

Any other advice for keeping sanity?

It very much like congestion of the epididymis to me in both of your
cases. Well, the "lump" might be something else - does it seem to be
part of the epididymis?

I had the same issue: a dull ache much of the time that was worse some
days than others. I seemed to get slightly better over time, but mostly
remained at the same low level on average. You can read my story on
David's Web site under "Giraud," but my uro said similar things,
specifically that some men have this lingering issue, and it can take a
long time (a year or so) to resolve. He also said that with some men,
it never resolves on its own.

The problem is pressure inside the epididymis due to the sperm build-up.
He offered to convert mine to "open-ended", and I finally took him up
on it after thinking about it for a few months. It was basically a
"redo" of the vasectomy on the testicular end without resealing these
tubes. I think he removed a cm or maybe a little more from the ends.
This set me back for a couple more months, of course, since surgery
"perturbs" things. But I must say I have improved over time very
gradually. I am not even sure this is because of the 2nd operation or
just because of time passing. I now hardly ever feel any discomfort.
So there is hope!

I know there is probably a small chance going in there again will make
things worse, but I am glad I did it.

Alfredo, I had an OE vas about 2.5 years ago. I insisted on an OE
based on what I had read - and the likelyhood of avoiding what you are
experiencing now. I'm glad that I did. If I were you, I'd find a Uro
who fully understands what is going on a consult with him. Conversion
to OE might be the fix, but you need to be sure. I've read that there
are problems associated with cutting the testicular ends too short.
Matter of fact, I know a guy whose vas was apparently done cutting too
close to the testicles and he wound up with a couple of problems, lots
of constant pain, and ultimately wound up having one testicle removed
due to aggravation with the epidydimus left on it. Be careful.

Hi

if this guy is from the UK then chances are its a closed one...

I recently tried to discuss options (earlier post that david kindly
replied too).

However the surgeon wouldnt discuss it - and when his secretary asked
him and got back to me - open ended wasnt on the menu - no matter my
preference.

So overall the jury is still out here in the UK.

If anyone has found a surgeon in uk/scotland that does open ended then
I'd love to get more information.
