Opportunity to participate in research

I am doing my dissertation on coping and cancer with newly diagnosed
cancer patients. To participate you need to be:
* Six months diagnosis and less
* 18 years and older
* Any type of cancer is acceptable
The survey will take you about 15 minutes. You can enter a raffle
with the possibility to win one of two $50 or have the money donated
to a cancer organization.

I tried to do this, but have come across problems. The design is USA
centric. Are you interested in data from Non-Americans? I am British
and living in the UK.
The question about education was not quite right for UK people, though I
was able to choose what seemed like an equivalence if I am correct.
Also I have a problem with your marital status question - I am currently
in a UK civil partnership (ie with a same sex partner) and we also live
with another partner (of the opposite sex) in a polyamorous
relationship. This is not catered for.
Your question about religion does not allow for the answer "none" - how
do you expect atheists to answer this question?
You also expect people to disclose their household income. I am not
willing to do this. Actually it would take some effort to work it out!
Therefore I have given up completing your survey after only the first page.
I do not think I will be the only person with issues and this seems
unfortunate, hence my sending you feedback about the design of your
questionnaire.
