serotonin syndrome?

Hi,
If an SSRI like Paxil, taken at full dosage, is topped up with
Cymbalta, how is it possible to avoid serotonin syndrome, i.e., too
much reuptake of serotonin?

I'm not sure, but since everybody reacts differently to different meds and
different doses, what if the Paxil is working but barely?
