DDS in Texas sent the paperwork for disability to Moline ?

The request was from the DDS in Austin however the questionnaire was
to be sent to Moline Ill. Any idea as to why? This was the
questionnaire for Psychological and work history for My wife. Just
wondering if Texas is that swamped.
