You're right... It's Kurt Cobain. The reason for the other spellings is
in the FAQ.

According to "Come as you are" (the biography), Kurt Donald Cobain is his
birth name and he fucked it up when recording the Bleach album. After
that, he thought well hell, why don't I just keep fucking people up. So
he did. (He also said in the book that he did it coz he wanted to remain
anonymous (as if it were possible)).
