My feet have spread - shoes rub against toes

My feet have spread in recent years. Now it is difficult to find
shoes that don't rub against my toes (usually it's my second to
last toe). It is especially a problem with dress shoes. Wider width
shoes don't seem to work because my heels are narrow and tend to slip
out of the shoe. Do you have any suggestions? Any specific shoes I
can try? Where can I go for advice? Thanks.
