ZAMBOANGA CITY, June 17, (PNA) - The Social Security System (SSS),
in its aim to upgrade its members, announces the availability of the
new tampered-proof identification cards for all its members,
dependents, and pensioners.

SSS Regional Manager Adelaida Drapiza today said the ID cards will
be issued to all its members who paid their monthly contributions
even for once only.

She said those qualified for the issuance of the IDs are those
persons who are receiving their monthly disability, retirement or
survivor's pension like spouse, parents, or guardians from the SSS.

Others who are qualified are children of the members receiving
dependent's pension from the SSS on account of his retirement, total
disability or death.

Drapiza likewise explained that those who would like to avail of
the SSS tampered-proof IDs must personally visit the SSS office in
their area for a photo session.

No proxies or substitute for the pictures are allowed, she added.

A member who would like to get his or her ID card must also fill
up the E-6 form which are readily available at the SSS office along
Veterans Avenue at the nearest SSS office in their area, Drapiza
continued.

She said a member securing his or her SSS ID must also present any
of the following documents: passport; PRC card or Seaman's Book.

In the absence of these documents mentioned, Drapiza said a member
may also present any two of the following: driver's license; valid
NBI clearance; school or company ID; postal ID; senior citizen card;
major credit cards; voter's ID; savings account passbook; alien
certificate registration; GSIS member record; certification from the
Southern/ Northern Cultural Communities or Office of Muslim Affairs,
and TIN card.

Those availing new IDs or replacing their ID card for reasons that
they lost their identifying fingers or have change their facial image
and those with change of name, date of birth will be free of charge.

However those with lost or damaged SSS ID card or those who will
make changes in the option to print their date of birth on the card
will have to pay some amount at the SSS office where they are
applying, Drapiza stressed.

All SSS ID cards will be sent to the member as soon as it arrived
after processing the same at the SSS Manila office, it was learned.
