Assignment Help Needed

I know this is way off topic. But I don't ask for help much. I read, listen
and try and help out when I can. But now I could do with a little help. No
selling, no spamming not even for getting bandwith used.

I have an assignment due in in January, Quantitative Methods. I have created
an online questionnaire and would be very grateful if you could fill it in
