This address is off a form I got several years ago. It should
be the same: MICHIGAN DEPT. OF PUBLIC HEALTH
OFFICE OF STATE REGISTRAR
3423 N. LOGAN STREET
P.O. BOX 30195
LANSING, MI 48909
There is a fee.

According to the CDC/US Dept of Health & Human Services Booklet
"Where to Write for Vital Records", write to:

Office of State Registrar & Center for Health Statistics
Michigan Department of Public Health
3423 North Logan Street
Lansing, MI 48909

Send a check for $13 - state has records beginning in 1867 or she
can write to the county of birth. Required info in request - full
name of record, mother's name (maiden name), father's name, birthdate,
gender, place of birth, relationship to requested record, reason for
request.
