Does anyone know the address/cost for obtaining a death certificate from
the state of Massachusetts? Thanks.

My current information from Massachusetts Archives, 220 Morrissey
Blvd, Boston, MA 02125 (their form letter response to my August 1
inquiry) is:
State returns of vital records (1841-1900), certified, are $3.00. They
want you to cite the volume and page numbers when ordering. (Since I
had included a check for two events, one of which they couldn't find on
the index, they sent my check back with the index information for the
death they could locate. They will research with names, dates and
places provided by you).

Pre-1841 records and certificates are held and only available from, the
Town/City clerk where the event occurred. There is no statewide index
for these records.

"State returns for vital records after 1900 are held by the Department
of Public Health, Division of Vital Records and Statistics, 470 Atlantic
Avenue, Boston MA 02210 (617) 753-8600. All requests for records from
that office must be accompanied by a $11.00 research fee."

By the way, their response time has been no more than 3 weeks the two
times I have used them recently.

The MA State repository is: Registry of Vital Records and Statistics,
470 Atlantic Ave., Boston, MA 02210-2224. Tel 617-753-8600.

You can also obtain vital records from the town or city clerk where the
event took place - probably quicker!

The 220 Morrissey address is for records 1900 and before only!

All birth, marriage and death records after 1900 are at the Registry of
Vital Records and Statistics, they moved and year or so ago to Atlantic
Blvd. in Boston. (I don't know the address) The cost is $8 or more, it
could also take a lifetime for a responce. So you are better off to write
to the City or town Clerk where that event occured. Each Clerk has a
different fee, some are $3, some are $6. it will also be cheeper and much
faster!

I have also been told the Boston Public Library has records pre 1841.
Call and check before going.

The Mass archives at columbia pt. (accross from JFK library has indexes
and records 1841-1900 and indexes that can give you an idea of what town
if your not sure. indexes from 1900-1971. open saturdays 9-3. they also
have books for many cities and towns circa 1850 and state census records
for 1865 and 1875 or 55 i forget which, or possibly more.

forwards, write to:

Each certificate costs $11.00. They will honor no more than five (5) requests per
letter.

As stated above, the earliest records on file at this address are for the year 1901.
If you want earlier records, you should contact the Clerk of the City or Town where
the event occurred, or, for the years 1841 through and including 1900 you can write
to:

These certificates cost $3.00 each.
