Nagrand Slam .. damn!

I have been going nuts trying to get the achieve Nagrand Slam, it's the last
I need for Loremaster of Outland

My counter shows 74/75 and every tip i've had has been along the lines of
'you've probably missed one of those quests that lead to nagrand' - although
I've gone through the wowiki nagrand quest NPC s and my count suggests I've
done more than 75 .. like around 79. And having the achieve for all the
other areas it seems unlikely i've missed one. I have my minimap set to
show low level quests and i've flown round Outland for quite some time and
picked up nothing new.

I even set NPC alert and waited for Kristen DeMeza but she assured me that
I'd already done that quest after I saved her ;)

Eventually it was suggested I may not have done Headfull of Ivory and the
followup .. now here's a thing. That can only be completed at lower rep
levels, and I am exalted with the Consortium :p

Apparently the quest A Mystifying Vision which sends you to Area 52 can be
lost as well if you take any other quest that sends you to that particular
spot. - I can't recall doing that

I've done the Howling Wind and Murkblood Invaders item-drop quests, Wazzats
Jump-a-Tron quests, the Nessingwary ones.. I have all the mounts from
Telaar, I've done the ring of blood .. throne of the elements - all done -
the only one I can find remaining is a PvP quest but that dosn't seem to
register for the achievement.

Has anyone else found themselves stuck with this one ?.. like really stuck -
I'd love to find one more quest to nail this place!

Have you done the escort quest at Sunspring Post? The quest only shows up
when you kill the two mobs guarding the prisoner. (There are two prisoners;
one for horde and one for alliance.)
