Dunno if it's possible - maybe there's a hak pak or something.. if not I
guess you'll have to make do with role-playing a character who doesn't look
like you with big ears.

Yes, but every person playing has to have your custom portrait
downloaded to their /portraits directory in order to see your smiling
face.

Any graphics program that can save uncompressed Targa (.tga) files
will work. Irfanview (http://www.irfanview.com/) is a free one.

Well, ok: download a portrait from anywhere, put it in your portrait
folder. Edit the pictures to keep size and file type but put in your
picture. Rename the files so you got a distinct file name for your new
portrait.
Choose the portrait when creating a new character.
And now the difficult part: get everyone else you play with to put
these picture files in their portrait folders so they see them, too.
