BG2 Copper Coronet sewer quest trouble

Hi all. I'm stuck on the sewer riddles quest. I've got the hand, the staff,
and the ring, but hell if I know where the fourth item is. I figured it was
the blood of a friend. Also, perhaps unrelated, I ran into a room where
mushrooms attacked me, but it appears nothing further is in this room. Can
anyone help?

In the eastern end of the sewers you meet a fellow named Qualo and he has a
friend who happens to be a carrion crawler. You get blood from this
creature by killing it. Rather sad.

The room with the Spore Colonies and Myconids I am not exactly sure what the
deal was with that. When I went in there was an earth elemental or
something that looked like one fighting with the Myconids. I dispatched the
Spore Colonies and Myconids quickly. I think I found some minor treasure
but nothing to write home about.

I saved him once. Didn't use Otilukes, just killed the myconids before they
could touch him. He dies anyway, guess he's scripted to die no matter what
you do. Rather odd if you ask me, perhaps its a leftover from an almost
quest that got yanked right before release.

Doesn't work. He dies whether the myconids actually touch him or not.
(Otiluking NPCs turns them hostile, also.)

I always thought he was scripted to die as any time i tried this and managed
to kill the myconids he still died, you should be able to recruit that guy.
Now would that be cool or what!!?
