Cast Stone to Flesh - there are scrolls around for this,
if you don't have a suitable spellcaster. (One reason
why I like to have _two_ healers, so they can cover one
another...)
IIRC there are a few character-interaction bugs involved
with restoring stoned characters (it may stall romances,
and/or do other weird stuff) but I haven't encountered
this for myself - I don't think I've ever _had_ a BGII
character petrified.

Then you are probably facing an Elder Orb (be afraid).
The cloak of Mirroring acts like a perminant minor spell deflection
(hmm, think thats right) so only effects spells of ?6th? or lower. The
Elder Orbs cast several spells that are higher then that so arn't
reflected, The main one to watch out for is imprisonment, a touch
spell, so if you find an orb floating toward you, don't let it touch
you :-)
Oh, and the spells remove magic and some fear spell are not effected
by the cloak, either.

It must be damn short-range then, because I have never had it cast at
anyone more than one hex away from the caster (but then, most monsters
only attack the nearest target).

What's with the Shade and Elemental Liches, anyway? Aren't
they good guys of Netheril? I think Bioware could have made us
feel a heck of a lot more guilty for whipping their bony butts
to "rescue" Kangaxx.

I suffered a bug when my pre-made Kensai got petrified. I turned him
back to flesh, and then he gave me some lines from Mazzy's dialogue
scripts! I've got this hulking half orc trying to impersonate a sweet
ikkle halfling lass, while we're surrounded by angry Beholders.
However, once I told him he could rejoin my group (the dialogue was the
one Mazzy gives after you choose to have her leave the group - the "oh,
why me, please don't met me leave. Will you take me back *pouts*?"),
there were no other problems with him, and he got his old soundset back
again.

I have a question about this. Did this happen in a multi-player game? Or
did it happen in a game that started out as multi (to allow the extra
character) then switched to single? The reason I ask is that I've played a
(solo) multi-player game, that was not switched to single, with a party of
six created by me (never actually finished it though), and whenever a
character was dropped from the group or petrified he would stand there until
I had a character talk to him. After talking to him, the character would
say something cute (like "Goodnight ladies, ladies goodnight") and then
vanish.
The reason this happens is that Bioware (they only know why) changed the
"dropped-from-the-party" dialogues of multi-player created characters so
that they vanish instead of asking to rejoin (they would not vanish in BG1).
The fact that you got the "do you want me to join again" dialogue makes me
wonder if it happened in a multi-p game that was turned into a single-p
game. BTW, the dialogue is messed up because the file is the very same file
used in BG1, and because under normal circumstances you should never get
that dialogue, they didn't bother changing the string references to point to
something that would make sense (and so you get some lines that would
normally be said only by Mazzy).

P.S. I've "fixed" these dialogues so that they make sense and allow a
player to drop homemade characters (w/o them vanishing) and later ask them
to re-join if he wants (if not, then they stand there, wherever they were
dropped, forever). If you, or anyone else, wants these files, then let me
know and I'll send them to you (they're three files that are dropped into
the override directory).
Imprisonment is touch range only (check Elder Orbs trying to Imprison
you). Kangaxx casts a special version of Imprisonment which is
unlimited range, supposedly to simulate his PnP demi-lich ability to
Imprison at will. For other such idiosyncracies, check out the Vampiric
Touch spell, normally a touch range only effect; the Bhaalspawn innate
ability of Vampiric Touch is of unlimited range however, and darn useful
it is too ;-).
I have actually not come across Kangaxx yet, at least not on purpose (I
did open a sarcophagus downstairs in the 'bridge' area, and wished I
hadn't - was that him?). What kicks off this quest - I've finished
Irenicus off several times and don't seem to have been sent Lich
hunting.
I also came back from spellhold via the Underdark - seems that I missed
some areas that way .. if I come back by ship, do I still get to play in
the Underdark alter on?

Well, that certainly sounds like an excellent way of handling characters
leaving and rejoining the group!
Ahem, back to sincerity now: it certainly explains why I got the weird
dialogue. However, for a game which supposedly had so much time put
into its multi-player side, it seems strange that nobody considered the
problems if a game was copied from multi- over to single-player mode.
Maybe an e-mail to Bioware is in order to ask if they are going to do
anything about this for Throne of Bhaal.

You're never "sent" Lich hunting. You run into the Shade Lich near the
blind guys in the Sewers. Perhaps that gets you interested enough to search
around. If you find Kangaxx himself he'll send you one a quest to defeat
his two "guardians".

Yes. You just miss the Shark People city. It has some cute dialogue, nice
scenery, a powerful magical cloak (and part of the Halberd of Uselessness,
plus part of the Vorpal Blade from Saemon). Then you move to the
Underdark anyway. If it's not too far after that, consider reloading to
take a look.

Actually, if you stumble on him in the docks district (near the bar, trapped
door), he'll send you after his parts.
The essence of the Kangaxx quest is that the old chap is in pieces. You
need to find the guardian liches and grab their hoarded Kangaxx body (?)
parts to assemble the demi-lich. IIRC, there's one guardian in the
Bridge district and the other in the Temple Sewers. Then the big guy
himself hangs out in the Docks, next to Mae'Var's Guidhall.

Yes. If you take the portal from Spellhold, then you go directly to the
Underdark. However, if you accept Saemon's offer of a lift on his ship
(well, it's not actually *his* ship, but you'll see how it works out
eventually), then you can access a new map screen (Sahuagin City).
There's a few nice items there, especially for monks and melee warriors.
Taking up Saemon on his offer of a lift is the only way you'll ever see
this map, though, as you can't go back and take a different route from
Spellhold. You will need a save game from the end of chapter 3 if you
want to visit te underwater areas. Once clear of the Sahuagins, you
slip back into the main thread of the story by entering the Underdark,
just as if you took the portal.
