Gods, I can't wait to get out of this place. The game is really
effective at creating environment; I hate being in the Underdark, and
I hate having to play Drow and do extended undercove work.

But while I'm here, it appears that I'll have my pick of the Mind
Flayer, Beholder and Kuo-Toa dungeons. Is there any reason I can't go
and just punk on all three, provided I definitely do the one I signed
up for with El Bitcho? It would be consistent with good alignment;
Kuo-Toa, Mind Flayers and Beholders all need as many asswhoopings as
possible, especially as I've got a paladin along.

No reason at all (well, except that obviously fighting your way through
all three will delay the time when you return to the surface). The fact
that you must go to at least two of the areas to assemble the Equalizer
suggests that the game designers rather expect you to. Have fun.

Nope, you can even do all three areas before you start Ust Natha (that's
how I did it the first time - I stumbled into the beholder and mind
flayer lairs, and well, the Kuo-Toa were in my way), just keep the
appropriate items to hand over to her.

Yeah, I cleared all three before given the quest and then went back
and beat up the regenerated replacements, several times. If you don't
want to carry the quest items around, you can stash them and come back
for them later when you get the quest.
