I gave the Book Of Mythals to the wizard in Kuldahar, and he says that he gives
me an item for a reward. But the reply when I do this says "no valid items or
responses" or something like that. Then I check my inventory and there is
nothing added. So, apparently I got the experience, but no item from the
wizard. I have the patch, so I don't think it's a bug. Has this happened to
anyone else?

That is a bug, one BIS hasn't fixed yet, even in their first
patch. My recommendation is to rob him blind(he's got an Amulet
of Metaspell Influence, and a Ring of Free Action).
