What is the flame of Udun?

What is the meaning of the "Flame of Udun" in the FoTR? I cannot find
it mentioned anywhere else? Does it mean that the first Balrogs were
made of fire? Where is that fire in Middle Earth?

Also, who did make Balrogs? Tolkien seems to have changed his mind some
times on this. What do people here think?
