I have had that happen. It could be a couple of things.
1) The DVR isn't getting sufficient (or any) signal to record a
program. This indicates a problem with your dish or (more likely) the
cabling from the dish.
2) The DVR is dying.
Looking at the recording history may be useful. From Menu:
Recordings > Manage Recordings > History
This may give you a reason for the epic fails.
You can try doing a red button reset (RBR) because the customer service
rep will tell you to do that anyway, but it probably won't work. (If
you don't know how to do that, just google it.)
You'll almost certainly have to endure a service call. They were here
twice for me about this. The first guy did a perfunctory job of
inspecting the cabling, and he swapped out the DVR. When that proved
to not work, they sent a guy who knew what he was doing and went over
everything thoroughly. He found critter-chewed cabling in the dish
itself and replaced the junction that had been attached to the back
wall of my house since 1999; the silicone sealer had long since
degraded and was allowing rainwater to migrate up the inside of one
cable to the junction inside my house, which took out the junction.
They called a few days later to see if everything was all right, and it
was. I haven't had a lick of trouble since.
