Meaning, no warning?
As the story goes, when he was in the car?

Isn't there also a story that it was a stroke, and not a heart attack? I
seem to recall either reading this or being told by someone.

Maybe that is what I remember. It is interesting that we don't see a
discernible change in Shemp's performance after his stroke (whereas Curly
was very affected by late 1945).

His hands always seemed to do that, even before his stroke, and it may
have even been intentional. Look at HOLD THAT LION in one part during
the file cabinet scene. I think it's when Moe says "I have to do
EVERYTHING around here!" where you can see Shemp's hands shaking.
There are some other shorts he does that in as well, but I can't
remember any other examples now.

"I'm in a hurry all over!"

I would bet dollars to doughnuts that it was an aortic dissection, the
same thing that killed John Ritter. Even today, there can be no sign of
it coming, and back then, they wouldn't have thought to look for it and
it would have been identified as a 'heart attack'.
Tommie's source of information is correct.
Sandie (born after Shemp's death) was only relating her mother Geri's
first-hand information. Geri is still with us; hopefully for many more healthy
years.
Geri was in the ER, with husband Mort and mother-in-law Babe that Nov. '55
night, and were told by the attending doctor that Shemp died from a cerebral
hemorrage.
The doctor who signed the death certifcate never examined Shemp. Indications
are, the death certificate is wrong.
