Is anyone else having this problem? Is it a setting in the new VM
system, perhaps?

I've run some tests, and am seeing the same problem here in Northern
California: if your phone is off-network (e.g., turned off) when a new
voicemail is registered, you won't get an indication of that voicemail when
your phone goes on-network. I consider this a fairly serious problem.

South Texas here.. same problem. I normally turn the phone OFF at
night to charge it, and so it won't disturb me. Discovered when phone
is powered up, no message indicator but there are often VM messagse.
Note that if someone leaves a TXT message, I do correctly get that
indicator.
A long call to Customer Service, after escalating twice, basically
resulted in the "take the battery off" shuffle. They would not give me
a ticket number for this problem, nor escalate it further. As far as
CS is concerned there is no problem. Did some tests BEFORE calling
those folks, and it's a problem with two phones I have (GSM), at least
1 friend's TDMA phone, and two other GSM phones.

SMS is different because the network is transmitting the message to the phone,
and the phone sets the indicator.

AMPS, CDMA, TDMA, Magic-smoke-in-a**-MA, etc., that this type of
problem cannot be solved.
When a phone is turned on, data is exchanged BOTH ways to provide the
phone service. Surely they should be able to, by 20 years in to
Cellular service, know how to set a bit or two to toggle the msg icon?

This appears to be a legacy of the voicemail system inherited from ATTWS to
which all subscribers are being/have been migrated.
FYI, Cingular is not the only carrier with this problem. Alltel's VM
notification has been unreliable for nearly a year (approx. based on my
crappy sense of time). During some online research I've been doing for the
last month or so, Verizon customers also have issues with VM notification.
It does appear to be related to whether or not the phone was turned on when
the message was left.
