Our musical trio uses a 4 channel powered mixer and we use all 4 channels.
How can we add a monitor speaker(s)? We have no line out or other outputs
other than 2 1/4 inch speaker jacks and effects in/out. The PA is a
SoundTech S40 system.

Get a Behringer Eurorack UB1202 for $79.00.
Run your mics to it first and set the levels there.
Send the main out to your SoundTech S40.
Send the FX out to a Powered wedge or wedges.

The real pros here will know whether this would work or not.

This would work. there are better ways.
fx out will be post fade so your level to out front will also effect the
monitor level (maybe this is not a problem for you)

Sure...especially if it has more busses (pre-fade aux sends).

Since you are dealing with only four inputs I'd "Y" the inputs to the
two mixers and designate the Yamaha as monitors and the Sound Tech as
front of house. A"Y" cable is simple to make with basic soldering
skills or a local shop could whip them up for you. You'll need a
female xlr to two male xlr's, the gain change doing this will be
negligible and the two mixers will operate completely independent of
each other. Be careful to apply phantom from only one mixer if it is
needed and keep both mixers on the same ac source to reduce the chance
of hums and buzzes. You could get four monitor mixes in a pinch from
the Yamaha using the left/rights and the two auxes. Now in the new can
of worms department you'll need some eq's, power amps or powered
monitors. It never ends...
