Below is a link for your invoice for the current billing period.  Payment must be received before 11/1/2023.

 

Please follow these important administrative tips: 

1.   Make checks and or money orders/cashiers checks payable only to: Cultural Group Benefits

 

2.Send your envelop with your check to:

    Cultural Group Billing Office

    6701 Upper Afton Rd.

    St. Paul, MN 55125 USA

3. Payment is due by the 1st of the Month.

**The Group payment is always due on the 1st of the month.  Payments should be in our office by the first.  Your Group policy grace period expires 30 days after the due date.  If your premium is not paid in full The Hartford is not contractually responsible for any death claims that have occurred during the grace period.

4. Removing Members

 

Removing Members: Option 1

 

 

 

If you have members to remove we need you to send an email stating the full names, date of birth, and effective date of the individuals termination.

 

 

 

Removing Members: Option 2

 

 

 

You may also note REMOVE and write the Effective Date of termination next to their name on the billing.  You must sign at the bottom of the billing and send that in with your check (don't cross out the members name)

Note:  Members will not be removed just by simply excluding them from any type of listing the group leaders send in or provide to the billing office.  We cannot remove active members who are on your premium billing list unless you follow either option 1 or option 2 above.   This procedure is put in place to help clarify the billing process and protect not only the Cultural Group Benefits offices but you as leaders of your group.  

 

5. Aging members

 

Please help remind us when an insured member on your group is turning 65 or 70.
