Suspension letter/form based off of log entry

I have a principal asking me about having a letter/form in PowerSchool that we can print out and have parents sign based off of a log entry in PowerSchool.  I was wondering if anyone had a procedure for this or something that is used in your school district. 

Principals note:

Our board policy says (and federal law) that students must have Due Process if we are considering suspension. And parents must be notified in writing. We have NO forms here at PH and I know HT has no forms either. Due Process is not the issue as each child has "their day in court" and based on that the student is either suspended or not. I'd like to have some conversation with ad min about putting together some forms to use. Long story short, could we add some suspension forms in powerschool?

Yes, we have something like that, Richard made it and I’ve tweaked it.

 

We have a link to print any log entry, and then if it is a suspension we have two special letters that can be printed (Incident Report and Due Process letter).  Our Log Entry page is also customized to who more information about the log entry.

 

 

 

You’ll need to customize your admin/students/log.html page to add a column for the link to print, then you’ll need to add the actual page that has the info you want to print.

Here are copies of our pages, which may be a bit more than you want, but I like our setup.

 

It takes a bit of work if you want to decode a log entry so that it displays the correct type or consequence, they are stored in the database as numbers, so you need to look them up to decode them.

 

I just recently changed our Log Entry page so that it does not show the entire entry.  Anyone who has access to that page can see the whole description of any log entry.  We decided that was not appropriate and wanted to limit what was viewable.
