Form K F C 62 B of March and Specimen signature of the DDO  to be send to the Treasuery Office\Sub-treasueary office.

See the first page , letter sent by Treasury HO. They sent a excel file to all treasuries for entering data. But treasury officers just give a printed format and say submit in both hard copy and softcopy on CD. I scanned it, ran OCR and prepped the format. Now, after reading that a excel format was available, I feel terrible : (
