Bug in postscript injection not working on second and onward page of
a print job is fixed. This affected HTML links as well as more complex
image or code injection.

The problem seemed related to an issue with more recent windows
versions not returning true for POSTSCRIPT_IDENTIFY if done after the
first page. I've put in a cache so that once it is identified as
a Postscript printer's HDC, it will remember.
