Each first budget proposal requires approval

Hi! 

We have built an integration via the Google Ads API to create and top up account budgets (via AccountBudgetProposal).

Some of our older client accounts (created manually via the Google Ads web UI by account managers) had individual service agreements. 
When we applied the first budget proposal via API, the proposal entered a “pending approval” status, and one of our internal payments users (in our company’s Payments Users list) received an email requiring manual approval. 
After that manual approval (by our manager), subsequent budget changes seem to work normally (got approved in minutes).

I would like you to clarify the following: 
1. Under what conditions does Google require a manual approval (by a Payments user) for an AccountBudgetProposal (especially the first proposal)? 
2. Is the manual approval always required the first time for accounts which had individual service agreements? 
3. Once the first proposal is approved, is there a guarantee that subsequent proposals will be auto-approved (i.e. not go into “pending approval”)? Or could Google again require manual approval in certain cases? 
4. Does the Account Budget Controls setting in the payments / billing profile affect this behavior? 
5. Can Google disable the requirement for manual initial approval for our manager account? If so, how do we request that?
