Multiple ads created when calling mutate once

Hi there, 

We're in the process of developing an application for keeping adwords account structure up to date when products change in a clients inventory.

We've run into a problem where we're calling mutate once to create an ad, but in in rare cases the API creates 2 ads instead of one.

Information about our application structure:

API version: v201509.

Using the adwords python library.

We're making multiple requests to the AdWords API at the same time spread across multiple workers (8 workers for our current tests).

We're seeing these errors on a test account, we haven't tried doing this on a live account yet.

These errors occur roughly once for every 1000 ads we create.

We are currently aware of some concurrent_modification errors we are looking into as well, which I don't think is related but I would mention anyways in case it does play a role.

Have you seen anything like this before?

We've spent a long time investing this now and are pretty confident the problem is not in our end, so we're hoping you can help us clear this out.

Do you have any SOAP requests and responses that show this behavior? If so, could you please provide them through reply privately to author?

If you do not have any, could you provide me with the ads Ids and their associated ad group Ids?
