Fail to submit the AdWords API Token Application

Hi
I recently created Manager account MCC in order to automate another regular active account via adwords api. 
Upload offline conversions is the main purpose why I applied for a developer token.
The accounts are linked, however, every time i try to apply the api token form, the submission fails and the form scrolls to that section:

We noticed your account doesn't have much spend history with AdWords. Keep in mind the AdWords API is meant to help large agencies and advertisers manage their campaigns at scale.
      - You must link your active AdWords accounts under the AdWords manager account (MCC) through which you are applying for a token by following the instructions here.
      - Make sure you have a fully functional website (not a demo site) so we can verify your company's business model.
      - Include thorough design documents (greater than 1 page) in the attachment section near the bottom of this form.


As said, 
- my mcc acount is linked with admin access to a spend active account
- websites are provided and getting traffic from adwords campaigns
- document is attached to the submission though it only describe a tool that is provided by google code here

I really can't see what it is I'm missing

It seems like I didn't make myself very clear in my question.
the additional form you suggested me to submit, is exactly the one i have problems submitting.
I cant see why is it every time I click submit is scrolls up to this section, instead of trying to submit my form:

Before proceeding, check your Developer Contact Email and Company Type in the AdWords API Center
     1. Log into your AdWords My Client Center Account and click the Gear icon
      2. From the drop-down, click 'Account Settings'.
      3. Select 'AdWords API Center' from the navigation tab on the left.
      4. In the section containing your Developer Details, click 'Edit'. 

Note that company type specifically describes how you will use the AdWords API. If your company type is incorrect, your application will be declined.

  • We divert traffic from adwords to our Landing Page where we compare different products/services and help the users decide which one suits them best. Conversion is made on 3rd party partners websites, and reported back to us on a daily basis.
  • Thats exactly where I would like to automate the process of feeding the conversions back to adwords using your APIs.
  • No - We advertise our clients using our own Landing Page, Hence all the ads we manage involved our own campaigns and LP.
