Add Lead form Extension - LEAD_FORM_MISSING_AGREEMENT Error

Hi,
I am adding lead form extension to campaigns, based on this documentation below.
https://developers.google.com/google-ads/api/docs/samples/add-lead-form-extension

But it returns the following error.
