*Mailing times are not included in processing times. Processing times only include the time your application is at one of our passport agencies or centers. The total time to get your passport includes both processing and mailing times. It may take up to 2 weeks for applications to arrive by mail at a passport agency or center, and up to 2 weeks for you to receive a completed passport in the mail after we print it. Consider the total time it will take to receive your passport when you are booking travel.

In addition to generating a completed application form, this website also creates a barcode that is placed at the top of the generated passport application form. This barcode contains the information gathered from the web page, and is later used to easily read in and process the information on your application. For the barcode to be generated correctly, all required fields must be completed at the time the form is submitted for creation.

If you fail to provide the information, you will encounter a delay in processing and/or denial of your passport application. You will also be subject to a $500 penalty enforced by the IRS pursuant to Section 6039E of the Internal Revenue Code (26 U.S.C. 6039E). All questions on this matter should be directed to the nearest IRS office.

This form should be submitted along with the DS-11 Application for a Passport, and brought with you when you go in person to apply for a new passport if your previous passport has been lost or stolen.

You can fill out the DS-11 form online or print out and fill out by hand. If you fill out the form electronically, at the end of the questionnaire you will need to print the form and bring it at the time of the interview.

After you receive your passport, if you change your name or find an error, you will need to get an updated passport to reflect the change. The process, cost, and forms you need depend on how long you have had your passport. Follow the steps from the State Department to change your name or correct your passport.

The forms you print must be clear and sharp. Applications that are blurred or illegible will not be accepted. They must be printed in black print on white paper. The paper must be 8 1/2 inches by 1l inches, with no holes or perforations, at least medium (20 lb.) weight, and with a matte surface. Thermal paper, dye-sublimation paper, special inkjet paper, and other shiny papers are not acceptable.

Some printers will reduce the photograph area on the passport application to less than 2x2 inches. Please make sure that the photograph area on the printed application form is 2x2 inches to ensure that the photographs you submit (also 2x2 inches in size) can be correctly attached.

DS-11 and DS-82 application forms both have 4 pages. Pages 1 and 2 provide information for the applicant and are not for submission. Page 3 requires information to be completed by the applicant, and although page 4 requires no information from the applicant, it does contain official legal statements and is required as part of the application form. Pages 1,2, and 4 should be read before completing page 3. Your application for submission must be printed pages 3 and 4. Your application will not be accepted without both these pages.

If you paid online, you must print your payment confirmation page and staple it to the front of your application form in the top right-hand corner. If you do not include your payment confirmation page, your application will be delayed.

To ensure safe return of your new passport please enclose a pre-paid (up to 500grams), self-addressed Royal Mail Special Delivery envelope. These prepaid envelopes can be purchased from any UK post office. If you are also applying for a passport card and would like to receive your passport before the passport card, please provide two envelopes at the time you submit your application.

This will ensure that the application process is accurate and complete and our office will process and mail to the correct U.S. State Department passport processing center.


Please note that two separate payment methods are required as stated above. The application fee must be paid by personal check or money order and made payable to the U.S. Department of State. The Execution Fee paid to the Acceptance Facility (Lucas County Auto Title) may be paid in cash, personal check, money order or all major credit cards.

Bring your completed U.S. Passport Application form DS-11 to any of our four (4) Automobile and Watercraft Title Branch offices listed above for processing, or visit any of our offices listed above and our staff will provide you with the required forms necessary to complete the application process. Do not sign your completed form. We must administer an oath prior to signing. We will take the required photograph(s), process the application, receipt all fees and mail to the United States Department of State's National Passport Processing Center. Remember, when applying for a U.S. Passport, you must provide a certified copy of your birth certificate and one other form of identification such as a valid driver's license or state I.D.

If you were born in the State of Ohio and are in need of a Certified copy of your birth certificate, you are no longer required to contact your county of birth. The Toledo Lucas County Health Department can provide certified copies for any of the 88 counties in the State of Ohio.
