Error Converting to tagged PDF: Knowledge Source Failed. <Find Spans> [15]

Has anyone ever encountered this error or this variety of error before? I can't find anything on it in the forums or on the web in general.

I got it converting a PDF prior to download to my Palm (in Windows).

This is just happening on one particular file - I've successfully converted numerous other PDFs (from the same source) w/o any problem.

The page it is failing on is not (visually) any more complicated than any prior pages that it seemed Ok with.

Any help or suggestions appreciated!

One of the users at my company got this error message....anyone know how to solve it?
Acrobat was unable to make this document accessible because of the following error; knowledge source failed find span 1 <find span> [1]
