Importing data to a form, from a data file for printing a form

I have a multi-page form into which I would like to import (merge???) data from an ASCII file. The completed form would then be printed. I work for a hospital and they have a system for capturing data which they would like to print on PDF forms. Multiple data records to one form. Ideas please. I am new to Adobe and the use of it.

I believe we may have spoken on the phone about this, but just in case, there are two Acrobat plug-ins that can help with this. Variform PDF or Variform PDF Pro.
