What's the easiest way to merge two pages in a PDF document? I'd like to create a A3-sized pages out of each two succeeding A4-sized pages.

With Acrobat, you can just print the document to a new PDF with the multi-up option selected in the Distiller or Adobe PDF printer options.

You might want to take a look at activePDF Toolkit -- specifically the StitchPDF method, which allows you to combine multiple PDF pages onto the one page.
