How to scan a form and create a fill-in form

I would use a PDF instead of Word.

I have about 20 hardcopy bank forms that I'd like to scan into PDF files and create fill-in forms for my users. What is the best/easiest way to do this? I tried scanning into Word documents but the formatting became very messy, and the banks don't like forms that don't look exactly as they want them.
Any help greatly appreciated.

Scan into PDF documents and use the Form tool to create each form field. The PDF will retain the page layout, so you don't have to worry about formatting the page or having items reflow if a text field is drawn a micrometer too large...
